I scheduled an appointment, submitted photos of the front & back of both insurance card and drivers license, completed the forms, & signed where necessary online. I got there to parking lot, texted the word “HERE”, & got a phone call asking me the reason for visit. I was told that I could to be seen by a physician so needed to walk inside the clinic. Everyone wore face masks. The office is a cool temperature so bring your sweaters & jackets to keep warm inside. I was seen on 2 different visits in September 2022. I was diagnosed & treated for 2 different medical conditions with 2 different medications. When I asked that my FMLA documents be completed. I was told there is a $50 fee and that they would not complete it because 2 different doctors had seen me. 1 was out of office. They told me to go to my Primary Care Physician to have FMLA document completed even though my primary care doc had not diagnosed or treated me for anything. My PCP was so busy that she was only able to schedule an appointment for me in mid-October. How is this responsible & logical?? Physicians Immediate Care soca had seen me, diagnosed me, k treated me by prescribing medication, yet they docked me around for 2 weeks about getting my FMLA document completed! I was just trying to keep my job secure while I was out sick after reinfection of leg after hospital discharge. Keep this in mind when you schedule your appointment. For small issues like a cold, flu, yeast infection, go ahead and visit. If you have need for FMLA document completion while away from work due to sickness and are being treated by Physicians Immediate Care, bring along $50 extra for fee, be insistent that they complete the form & FAX it to your employer. Reminder to have them remove all sticky Post-It notes before they FAX. Twice before in August, they did that to me and I had to go all the way back to office to make sure those sticky papers were removed & document FAXed to my employer so that my FMLA could be read and approved.
