This hotel was amazing, I had such a spacious, and clean suite with a beautiful view of the beach. Staff was outstanding and friendly. Ricardo was so pleasant and helpful helped get us checked in informed us of all the amenities and was so friendly. I enjoyed my stay it was a well needed.

The junior suite was sparsely furnished, the 39" tv was 10 years old and there was no room service. The king bed was a single hollow, you kept rolling into the middle, a disaster. Never again

The Costa Hollywood Resort is approximately 35-40 minutes by car from the port in Miami. The Fort Lauderdale Port is closer.
No there are some rooms without. Of course those that have them cost more
I believe the resort is smoke-free The room we had was smoke-free
The pool is on the rooftop. It's amazing. The best pool of the area. It has also a bar and incredible views. You have to use the key on the elevators but i'm pretty sure it's open to the public bwcause of the bar (not to swim or to use the pool).
From what I understand of the hotel/residences. It is a managed condo type place. People own units, then when they are not living in them the management company rents them out as hotel rooms. Most of the units have kitchens or kitchenettes, leading me to believe these are more for long term travelers I'm guessing room cleaning is done once a week. This is the first location that this company/ chain has done on the east coast as they are 99% in California. The pool on the roof, building design, rooftop bar, valet, and other amenities they have, that's why it might be rated 3 stars, not on the type of service you get.
From the valet elevator to the broadwalk it is a 2-3 minute or less walk, depending on ypur walking speed.
