Depends on which exam station you go to.
The test consists of 40 multiple choice questions taken from the Digest of Motor Vehicle Laws study book so the time would depend on how well you know the material. Typically doesn't take more than 30 minutes.
Any BMV agency (Xenia, Beavercreek, or Fairborn) can transfer plates with an Ohio title and Ohio Identification
In the summer, all days are busy since they have driving test and teenager applying for 1st time license. Every summer, I always had to be there for at least 1 hour regardless what day it was

The permit tests are walk in only. The Exam station tests Tuesday through Friday from 8am to 430pm, and Saturdays from 8am to 2pm.

I believe so. You would probably be charged for lost or stolen fee. Because you would be issued a new license. However, if it time for you to renew, you just pay the renewal fee.
