All you need to do is fill out Form DS-82 (you can also complete the form online and print it out) and take it, your current passport, a passport photo and the applicable fee (currently $110 for a passport book and $30 for a passport card) to Monmouth connection. It takes a while, like 6 weeks the government will mail it back to you.
They do notarize. Not sure about the will.

Passports re going to cost anywhere between 80$ to $189.65. If your processing for a minor (Under 16yo) its 80$, for new or a renewal. If you want to expedite its going to be an extra 60$, Expedite with overnight which is usually about 3 weeks, could be less, but most often guaranteed by the 3 week mark, --if in fact it does not arrive (you) would need to call the National Passport Issuance Center, Otherwise an adult passport is going to be 110$ for routine processing, ALL that being said, that is just the fees that the state department commands for the processing of a single passport <80/110> If you want to process in house, it is 25$ for now, come april 1st it changes to 35$ because of rising costs and for validity. pictures are done in house for 10$ for two biometric pictures(this means that, if you havent been introduced into biometric system otherwise, once you take that passport picture, you are now in the biometric system. (there is no way around that)
