She needs to sign over her rights at the department of social services and put you down for guardianship and they will send it to the judge to evaluate the case u will need to make sure that ur house is approved by department of social services and u have enough space and u need to provide proof of income and u don’t need the government for any assistance once u do a walkthrough with dad n a guardian at lighten come to ur house and talk to everyone that’s in ur home the judge will make a decision to place this child in ur custody permanently

If you are just wanting blank forms to fill out. You can go online to www.sccourts.org/forms and download or print them from the website.
