Yes. And you will need the minor's birth certificate and the gov't issued ID of the parent, as well!
If they have time we did walk in for 3 tattoos just a bit of wait if they happen to have an appointment highly recommend them though.

State of Nevada does not regulate tattoo or piercing shops. Clark County (Las Vegas) does have an ordinance on tattooing. Each establishment may have their own regulatory restrictions regarding minors. I recommend to give them a call.

Kyle Mahathey there nailed a Wood Duck as a cover-up over a 40+ year old tattoo. You can't go wrong with him.
Your best bet is to go in and talk to them. Describe what you want and they tell you a price.
