My father passed Jan 2021. However, we noticed his date of birth was wrong on the death certificate and immediately notified the funeral home. I waited several months hoping to receive a corrected certificate but have not. I then contacted the funeral home again and they stated they submitted a form for the correction back in March 2021. I then took it upon myself and contacted vital statistics in Jacksonville. I completed the form and paid the fee. Can someone from vital statistics assist me here since I haven’t gotten anywhere over the phone or through the postal service? UPDATE!! Everything was resolved. Thank you!!

When you go in, tell the person at the front desk you need your birth certificate and they'll give you a request form to fill out while you wait. Then when you are called up give them the form and they'll need your ID and a form of payment.
