They closed the the office for hardship license in SW Florida, you have to contact the office in Fort Lauderdale, the form for substance abuse treatment is a form that you can get at the dmv, have your treatment facility sign it and return it to the dmv w/o it they won't give you your license.
You will also need driver license records, police report, proof of address (not considered id) and picture id. If you are missing one thing then won't enroll you.
