You have to use the website and call the number and speak to an Representative who will provide you the number to assist with your needs.
It is insanely difficult to get through to a human being. They do have days that you can come in without an appointment. I would definitely keep calling them every day. That's what my husband had to do. It was a nightmare!

Yes you or your child's mother can carry them or even someone who has been taking care of your child financially For a while or that following or current year. But most of the time you should be careful because if it's new to them they will think it's fraud and you will get audited so if their is someone else besides you that is taking care I would get notarized documents saying that this person has took care of your child this year and a statement from your self or the mother saying that you give them permission to carry your child because they have taken care of your child's Financial needs
