I died ⚰ literally twice & have been denied 4x times already. The judge states and I quote, "applicant does appear to have all signs of mental disability, bipolar disorder, manic depression disorder, severe anxiety & panic disorder, PTSD, COPD, Asthma, night tremors, applicant seems to speak in an unknown Possibly forgotten Latin language & able to move objects clear across the room shattering glass ware & bending medical tools without ever touching them physically. Several nights during sleep observation applicant was lifted up into the air several feet then placed back down. He suffers from severe insomnia, remedy is to continue sedatives (Ambien & Xanax) for night tremors. Patient does show some mental challenges but is not in any clear present danger. Conclusion does not qualify for SSI nor DSS. Patient is welcome to reapply. Applicant under observation shows that although he does have some mental limitations it is my professional opinion that the applicant will be just fine in public society" end quote. Judge White W.P. NY Final judgement.

I'm fairly certain they have lost all of my documents. I sent my ID, birth certificate and marriage certificate through certified mail (which is the only way to file an application for a name change with this office's COVID rules) one month ago. They still have not opened this envelope. I have been calling, and they just keep saying "it's not in the system" even though the USPS confirmed that the documents were delivered to the office. This is absolutely unacceptable and now I'm pretty sure I'm going to have to go through the process of recovering all of these important documents, all without having the name change completed. This has all meant that the name on the deed to my house will be incorrect, which I was assured by this office that they would be able to change my name in time because I was giving them over a month to open an envelope before my closing date. Awful.

No appointments necessary. An intake computer, with a touch screen, asks basic questions, then gives out tickets and puts you on cue according to your needs.

I did not have an appointment at the New Rochelle or at the Yonkers offices. For any other office, I would either look online or call just to be sure that you don't waste your time. If they do accept walk-ins, I would definitely get there when they open or the lines get long.
