First time in this lounge. Good experience - quite some food/drinks selection. Not too crowded. Seems a bit small? But I didn't have time to explore the whole area yet.

at the lounge itself at the reception desk.
If I remember right there's at least one shower Suite in the MIA Sky Club. Your ticket alone will not get you access. You can buy a day pass or use your Amex if it comes with that benefit ....only International first class gets Sky Club access with the ticket
Of course, but they're not cheap. If you travel a bit, it is much better to buy an annual pass. Relaxing, nibbles, and folks that can help you when your trip is a disaster. (Esp. when the gates are a cluster. )
