Your best train(s) to take are the Coney Island / Brighton Beach bound Q or B train(s) to Sheepshead Bay. The B is better as it is express. Once you're at Sheepshead bay, transfer to the Coney Island bound B36 bus at Ave Z and E15 st and ride it for 5 stops, getting off at Ave Z / Ocean Pkwy. That's because there are no weeekned outpatient clinics... The clinics are open Monday to Friday only from 9am to 4pm

The office is on the first floor when you walk in the entrance all the way to the the end of the hall to the left of the security desk. You could also just call the main number and ask to speak to the patient relations office.

Labs will most likely not give out patient infoike that.. What you should do is contact medical records.. Call the operator and transfer your call to medical records.. As a patient you can access your own records and they can best assist you with giving the information you want.
