Your spouse has to go with you. Take two forms of ID. Birth Certificate or Social Security Card and Drivers license.
You will need a Military ID/Dependant ID to get on base, after that depending if you are the Sponsor or Dependent will vary on what is needed at the ID Facility. I highly recommend to always go with your sponsor, will save you trips.
You will need two forms of ID for you and your family member. They must be valid. If you the sponsor is not going to be there then they will need to have a da form 1172 and two forms of valid id.
You need to bring your old id and his death certificate in to the ID card section
