Proof of Social Security Number (i.e. Social Security Card, W2, Pay Stub, etc.) Photo ID (i.e. Driver's License, State ID, School ID) Proof of current local address.  Examples include: Mail addressed to your name post marked within the last two months.  Electronic version of a current bill or account statement showing your address viewed on a mobile phone (i.e. bank statement, utility bill etc.) Names and dosages of your current prescription medications. Names and dates of any immunizations received within the last 12 months. For any tattoos and/or piercing in the past 12 months, we will ask the date and place of business.
