All minors, including newborns, must have their own passport. In addition to complying with the general passport requirements, please note these additional requirements: • The minor plus both parents must appear in person • Original Birth Certificate to show Proof of Minor’s Relationship to Parents/Guardians for a minor under age 16 • Parents’ identity such as valid driver’s license or passport • If one parent can't be present, you need a consent form (DS-3053) from absent parent giving permission to get the passport. A copy of whatever the notary used as id for the absent parent (valid driver’s license (front and back) or copy of valid passport) • Minors age 16 or 17 must sign their name on the application • Minors under 16 cannot sign their name.
