Only services dogs sre allowed in the restaurant. There is a common outside area you and your dog can enjoy a meal.
I'm not sure which "crispy" breads Costco used to sell but if you mean crusty bread then they have a number of good option for loaves as well as crusty rolls.
Usually, but don't have to. I believe they pay servers a living wage in SF.
