Establishing care at the ID clinic is fairly easy. To be qualified for care, you need to prove your identity and residency (Driver's License/state ID with valid Georgia address will do it; if you don't have that, a current utility bill with GA address can be used) You will need Income Verification which, depending on your situation, could be paycheck stubs, W2s, a wage statement printed by the GA Dept. of Labor, a notarized statement of support, or some combination of these. You will also need proof of your diagnosis, but you can also secure this during your first visit to the clinic. Call 404.686.8114 and follow the menu options to schedule an appointment as a new patient (I think, but i'm not certain, that it's option #1 on the 1st menu, and option #1 again on the 2nd menu). If you need a social worker assigned let them know when you call, and they will get you set up. It may be worth double checking with the scheduler what you need to bring to your first appointment.
