Patient would need to renew every year, per state law. The initial consultation is $250, then there will be a renewal fee every year, which has not yet been established, but would definitely be cheaper than the initial visit.

The certification evaluation would be $150 as that is our discounted price. The actual card comes from the state so you would need to call and ask them if they have assistance programs with cost for that.

You will need your ID or DL that verifies your current MO address, your medical documentation that states your qualifying diagnosis and the evaluation fee.
