Hello John, I understand your frustration. I have been in the same boat. I was applying for a new ssn and there were tons of documents that they asked, I wasn't much frustrated with the doc submission but the time frame. Well, if you are looking to have a replacement card, I reckon, you can submit a request online.
I do think so but have all relevant marriage/documents as far a divorce decree/court order so you don't have to back. Have some money on hand to just in case you never know now a days.

Yes you can make an appointment. As suggested go there rather than call (faster) and the guard will give you one. Appointments are called on first. Keep in mind, there is a GOVERNMENT SHUTDOWN until who knows when. They are NOT open today, I was there.

I don't think so you have to go there and get the address where you mail it to a payment center out of town
I believe if you can obtain a copy of your S.S. card... I would think... that you would be given the actual S.S. card (IDK). Although I would, if I were you, take advice from the previous reply message from Krystal F., "You can ask for a receipt to prove you've applied for one and bring the receipt to any location you may need it for." Hope we helped.
