I got sent here for fingerprinting for my company. They were all super great, friendly receptionists and excellent staff!

It's the DC area, so of course when I made my appointment, the first availability was 3 weeks out. Upon arrival, apparently the ONLY person in the entire facility that does fingerprinting called out sick. I received no notification that my appointment had been canceled. To reschedule, the next availability is another 3 weeks out. This facility seriously has staffing issues so if you have a choice to use anyone else, then do. The staff was not helpful. The phone number provided to call to reschedule did not even have an option for rescheduling fingerprinting. The staff just shrugged their shoulders and proceeded to ignore the problem they created.

Not sure about the DC office, timing only allowed me to go to VA location. You should be able to call them to verify.

Passport and/or birth certificate. If you've changed your name since birth you have to bring a name change document. Ex. Marriage certificate.
