Avoid this insurance company. I had an intensive knee surgery and after reaching my max out of pocket of nearly $9,000 I was still required to pay for additional appointments and exams that year. Worst customer experience of my life. I spent over 40+ hours on the phone trying to explain my situation throughout the year
