Yes, it's, already went 3 times to this office and my Visa was approved, don't worry all is okay
It depends your nationality, in my case paid 120 usd for my Visa ( Mexico passport) but also you have to pay for the invitation and Invisa fee, I paid 220 total and waited 2 weeks to get my Visa.
Put Washington DC center. You should be able to print it still you might not have the correct form
Yes, you have to pay a new Visa fee but the amount depends on your country.
