Our experience with this home was terrible. Receptionist on the weekend that volunteers and has a terrible attitude and makes it difficult to visit your loved one. My mothers items went missing often and the worst thing they did was sent her to ER and not notify us and when they finally decided to advise us they sent her to ER and we called ER to find out our mother passed 7 hours ago! We could have been with her at the hospital or wait outside to get word and instead no one notified us we were living our lives as usual - we could not see her because she tested positive for covid however she was viviste daily before getting covid. We called daily to see how she was only to be told she was fine and made us feel like we were annoying them for calling to check on her. We asked if we could viviste by her window and were told no … if we knew she was not well we would have taken her home to spend her last few days with us regardless of her covid status … not knowing your mother has passed or have been taken to ER til 7 hours after is absolutely unacceptable! And we requested medical records and had a lawyer request them and to date we have not received them. This was back in December 31, 2021. I’d give them 0 stars but app won’t let me

I was looking for a SNf and had questions about the facility. I speak English and could not understand what they were saying. They did not actively listen to me and my concerns. I was told not to come there if I was going to be difficult. Good enough I won't go. Check them out before you send your loved one to them. It seems like they just want your money.

The BEST part of Wood Lake is the new activities director Amy Shiner, she has brought a Splash of color to a drab grey room which is now packed with happy, adoring residents! She is young, enthusiastic, passionate about her residents, caring, kind, and compassionate. The best is yet to come with this amazing young woman at the helm. Thank you to Ms. Shiner for restoring hope and laughter to ALL the residents.

This depends on whether your doctor has privileges in the center where you are admitted. If he or she is not credentialed for your facility, then your case will be referred to another doctor who is on staff there.
Generally speaking, a skilled nursing facility is a clinical care provider of 24-hour licensed nursing. A SNF , as they are often referred to, is primarily engaged in providing services for residents who require medical or nursing care and/or therapy services for the rehabilitation of injured, disabled, or sick persons. Care typically includes: rehabilitation, intravenous therapy, post-surgical stabilization, pulmonary management, and wound care. Additionally, facilities provide assistance with administration of medications, and aid in a variety of daily living needs, including dressing, bathing, walking and eating.
Social services staff works with our residents and their families during times of transition. Our team provides information, addresses concerns and requests, and helps plan the discharge of each resident. Social Services can arrange professional services for any of the following: vision, dental, audiology (hearing), podiatry, psychosocial/psychiatric evaluation, creation of advance directives, dementia assessment, and individual therapy.
Physical, occupational, and speech therapies take place throughout the week. You will be evaluated after you arrive, then a personalized therapy plan will be created to best meet your needs. Wear tennis shoes and comfortable loose-fitting clothing.
Our activities and programs are created with residents in mind. We plan activities that suit the needs and capabilities of each resident. We find that this helps the healing process of our rehab patients. Activities include fitness programs, singing, trivia and games, religious meetings, musical performances and other entertainment. Families are encouraged to participate with residents. An activity calendar is provided so residents can stay informed on upcoming events like parties, movies, outings, ice cream socials, etc. As always, family and friends are invited and encouraged to attend.
Prepare an inventory list of items you re bringing with you. -Medical records -Copies of living will and power of attorney -Copy of Medicare/Medicaid or insurance card -Several days worth of comfortable clothes, sleepwear and toiletry items -Something from home that might bring you comfort like a favorite quilt, photos, reading material, etc.
