This place is terrible. I worked there for not very long and just in that short time i seen how horrific management was. Heather Malaga has offered other staff Xanax pills and hit her vape right in front of residents and family. Other workers yelling and fighting with each other, all of first shift on their phone at the nurses station not paying the residents any attention at all. Very terrible place and state needs to be notified immediately.!

Hello,  my name is Herbert Green and I'm a registered nurse that used to be the assistant director of nursing at Trinity elms health and rehabilitation. Last year, Tonya hilliard administrator at multiple instances had explicit knowledge of my active symptoms and my close proximity to a covid positive individual, but still wanted to expose me to residents and staff at the facility in Clemmons NC. I have documented evidence of providing my concerns and symptoms to both Tonya and the Director of Nursing Jeanne Smart, who regardless of their knowledge proceeded to try to get me to come to the facility for testing and to work in direct contact with everyone there. I raised concerns on many occasions in person and text and was fired while under quarantine for performance issues. The public deserves to know the wilful endangering tactics the leaders of this facility utilized,  especially when it comes to their employees,  family members and residents.

This place was great several years ago, but that does not seem to be the case now. No fault to the staff there, it is 100% management. I received a call from my grandmother who was hysterical, she was there for several hours. No one checked her vitals, the nurse didn't check her arm band to verify who she was before giving her Asprin, and she was told she cannot have a potty chair for two days. A patient was standing in the hallway alone crying for 30 min.. Unacceptable. Pay your staff more or treat them better if you have shortages. Management needs to be fired IMMEDIATELY and replaced by staff that has a soul. Do NOT sign the consent form they offer where they would bring in 2 independent attorney's if you had a concern it prevents you from bringing in your own. Thankfully I work in the field and have called people who can intervene on a major level but I will not rest until upper management is gone.

Trinity Elms was wonderful to my mother in her final years. Everyone there went above and beyond to care for her. I cannot say enough good things. Thank you from the bottom of our hearts for loving our mother. Felton/Miller families
