You have to go down to the DVM in person and take I believe 2 forms of ID( like a PASSPORT, SS CARD, BIRTH CERTIFICATE). Ps. They are only taking people by appointment.so you will need to setup an appointment through the dmv website for Chemung dmv which can be as much as a month away from the date you decides to make that appointment.
