Go to the front desk in her office and tell the receptionist that you wish to become a patient of Dr. Deepa Subramanian if she accepts your insurance. If so, then a questionnaire will be given to you to fill out at home, or in the office if you have all the information asked for. You will then return the papers before your first scheduled appointment.
No unfortunately the office is not contracted with Medicaid at this time.
