Good morning. Let me start off my saying the staff is great! Customer service is outstanding, The experience I had there was over the top, I was welcome with open arms.Matthew was one of the staff members that I deal with I just love him ❤ 😀 just not to brag on him but the team is awesome. I would recommend autumn 🍂 leaves to anyone that's looking for a family 👪 place. For rehad or stay.

Your staff was wonderful and pleasant to work with and always had a smile on their face. I'm at the Autumn Leaves facility 2-3 times/month. The staff there is always friendly, smiling, and clearly love their residents. They work so hard to make sure everyone feels at home and has all the resources they need. The residents love the meals, and line up outside the dining room to socialize as they wait for the room to open. The facility itself sits in a great location, with a beautiful view of the park and White Rock Lake. I'd gladly recommend Autumn Leaves to anyone looking for a place to land in retirement!

This review is long overdue. I put my mother in there on May 17, 2021. I wasn’t given any kind of welcome packet or advised of any rules when she arrived. When she changed rooms a month later I found a notebook in the nightstand with this valuable information. Staff always seemed to be spread very thin, a problem most facilities had during the pandemic. Most of them were caring and attentive, but when I had questions or problems and made verbal requests, They were never addressed. I finally decided to document everything and communicated with everyone by email. The Director of nursing at that time ignored my emails for two months. It took two months to get mothers nails clipped, and the clippings were left on the floor of her room. A nurse came in shortly after I discovered this and said it happens all the time. An odor a very strong urine in her room happened several times. Because mom was a fall risk and was admitted to Autumn Leaves from the hospital where she’d been diagnosed with a bladder infection, which makes old people fall, I bought special yoga socks for her with high-grip soles. Despite asking the DON and many nurses to use ONLY those socks, after visiting and finding her with regular socks on, even after she fell again twice, they were never consistent in using them, so she wasn’t as safe as she could’ve been. Her mind declined rapidly after she was admitted and I asked on many occasions that she be evaluated by psychologist. After repeated request, the staff did an in-house evaluation. It took months for her to be assessed by a psychologist and I had a lot of problems getting a copy of the report. Last month I finally went through all my emails and compiled a long list of many problems that I repeatedly reported that weren’t addressed. I won’t list everything because the review would be huge. Shortly before I transferred her to another facility, a new Director came on board and things seemed to improve. I don’t know what it’s like there now, six months later, but I advise everyone who has loved ones there to check on them frequently. The building is so old that one day I arrived to find two rooms next to her taped off with a sign warning of asbestos. Although I asked to be informed of what they were doing for abatement, no one responded. Quite a few permanent residents had to move out, it was so bad. The flush mount light fixture over my mothers bed had dropped down about an inch while these rooms were taped off, which exposed her to the asbestos. I reported it and it was quickly fixed. I learned that when I took pictures and sent them with my emails, the response was marginally better. The air conditioning was also out for several days and portable units had to be brought in. On day one I asked that maintenance check her television because the picture was so bad you could hardly see it. The maintenance guy was apologetic and said that their entire cable system was like that. They were waiting for approval from the corporate office to fix it. As of mother‘s discharge date, 9/3/21, You still couldn’t watch TV. I had to ask that a trashcan be put in her room several times after they disappeared. She doesn’t like eggs and won’t eat them, but it took a solid month for her to get oatmeal every morning. Again, had I not been there every other day, these problems would not have been addressed. She had so many problems with the laundry taking too long to wash her clothes and losing a lot of them that I finally began taking them home to launder. Patients were notified in a letter put on their bed of any Covid infections. The number of infections was not posted anywhere, and family wasn’t notified. Mom‘s insurance didn’t cover long-term care, so we were private pay at about $6000 a month. She didn’t get the care she paid for. I transferred her to Cheyenne Medical Lodge in Mesquite and am very happy with them. There’s a world of difference between them and Autumn Leaves. At Autumn Leaves I always felt like I was fighting for something all the time and it was exhausting.

Visiting hours are 24/7, but we ask that visitors respect the resident's need for rest. In skilled nursing, visitors require a visitor badge.
