This location is inside of St Joseph's Women's Hospital. It is down a small hallway near the cafeteria. When I walked up to the window, the lady didn't notice I was there as she was looking at her cell phone. I said, "hello" to get her attention as I didn't want to be creepy and just stand there looking at her. She turned and looked at me as if I was inconveniencing her. She asked what I wanted and handled the transaction with indifference. We waited probably 10-15 minutes for the documents that we needed, which wasn't horrible. Parking for the hospital was very full in the lot right next to the hospital. You may have to park in the lot across the street from the first lot.

Unfortunately, a police report for stolen ID does not entitle the individual to free copies. All certificates issued in the State of Florida require payment at the time of issuance. Regarding Birth Certificates: Having a police report can substitute as one of the 3 required forms of secondary ID in lieu of providing a photo ID. If a client is unable to provide a photo ID (primary ID), then they are permitted to present 3 forms of secondary ID. These include utility bills, social security card, voter card, W2, bank statement, car registration, marriage license, rent/lease agreement, health/life insurance policy, military discharge papers, fishing/hunting license, or letter from another government agency (WIC, SS, traffic ticket, court documents, etc.). The police report of stolen ID would count as 1 of the 3 secondary ID types needed to issue a new birth certificate.
Either perform an obituary search on a news site like www.tbo.com, or request a death certificate without cause of death (assuming that they are not related to the decedent) for a fee of $10 from the Office of Vital Statistics. This is only for Florida deaths occurring 2009 or later. If the death occurred prior to 2009, then the record must be requested from the State Office of Vital Statistics for a fee of $5.
