If you need to view and/or print duplicate statements, log in to your myCalPERS account to access your statements. We apologize for any inconvenience this may cause. Good Afternoon Denise, If it’s your Annual Member Statement you can access that document through your myCalPERS account, but that does not tie into your taxes. If you are a retiree receiving a pension, you would need your 1099-R form which is also available in your myCalPERS account. If neither of these are what you’re looking for, would you be able to provide us with a little more information of which document you're referring to? Maria, you can walk in the form to the office and they forward it up or you can mail it to the headquarters location. If you mail it to San Bernardino they just mail it to Sacramento
