Hi William, Thank you for reaching out to us. Yes, if your wife has been locked out of her account, the best route would be to fill out a contact form. You may access the form by visiting our Questions, Comments & Complaints webpage: https://www.calpers.ca.gov/page/contact/questions-comments-complaints For the subject you can select “Technical Help With Our Website.” Fill out the remaining fields as prompted and we’ll respond to general inquiries within 5 business days. For faster service, give us a call at 888 CalPERS (or 888-225-7377). Please let us know if you need any additional assistance. Thank you.

How do I see my monthly Earnings/deductions sheet like I used to get in the mail? I also want to talk to someone about my Long Term Disability policies. Thank you.

CalPERS San Bernardino Regional Office | Jan 25, 2022

Hi Wayne, Yes, due to Covid-19 and until further notice, the State Controller’s Office isn’t printing and mailing statements for payees who are enrolled in direct deposit . If you need to view and/or print duplicate statements, log in to your myCalPERS account to access your statements. We apologize for any inconvenience this may cause. Additionally, you can reach out to our contact center to discuss your Long Term Disability policies. They're available at 888 CalPERS (or 888-225-7377) Monday - Friday, 8:00 a.m. to 5:00 p.m. Please let us know you have any additional questions. Thanks!

I need a copy of my award letter for the year of 2020. How do I obtain this for my taxes?? Is appreciate an answer ASAP. Thank you Denise Rhoden

CalPERS San Bernardino Regional Office | Aug 23, 2021

Good Afternoon Denise, If it’s your Annual Member Statement you can access that document through your myCalPERS account, but that does not tie into your taxes. If you are a retiree receiving a pension, you would need your 1099-R form which is also available in your myCalPERS account. If neither of these are what you’re looking for, would you be able to provide us with a little more information of which document you're referring to? As a reminder, our call center is always open for your questions. They can be reached Monday-Friday (excluding holidays) from 8AM-5PM. Their number is 888 CalPERS (or 888-225-7377). We look forward to hearing back from you.

Maria, you can walk in the form to the office and they forward it up or you can mail it to the headquarters location. If you mail it to San Bernardino they just mail it to Sacramento
