Not worth it, it's a walk in walk out cave.

There's no charge; the cave is within Fort Leonard Wood. You have to be able to access the fort. Drive to the parking area and walk down to the cave. There isn't a guide or anything like that. The cave doesn't go that far back (at least, that I know about), but you should wear sturdy shoes, a jacket, and carry a flashlight.
It's on Fort Leonard Wood so yes, once you are on the installation there is no other gate or anything by the cave.
Yes, but be careful of the edges of the cave and trail. It can get steep and slippery on the rocks.
