I had the same problem and I went to the DMV office and they wanted to charge me for late fee I told them that I mailed but have not gotten it back told me I can pay for it now and I did two days later they cash the check that I mailed and cause bounce check in my account had to go back to the DMV office to fill out a form to get my money back ii fill frustrated

That won't work. You need your SSN card, birth certificate or green card/citizenship with 2 forms of address. This is what was needed for the real driver license or ID card. Good luck!

You need to do a police report then take that to dmv to get a replacement. No appointment needed. Had that happened to me but quick tip if your Registration is due pay it as you cannot use the registration paper dmv sent in the mail once they give you new license plate. You have to go in person to pay the registration fee. Hassle but I learned my lesson as no one told me til I went to pay at kiosk and it didn't work. Had to wait again in line to pay Registration on different day though.
