I called and asked for my medical records and was told they'd have to get them out of storage and then could not release them to me because I no long reside in CA anymore. Then I was placed on hold waiting for the supervisor only to get sent to voicemail when the receptionist said she was there.

Sharon please drive to the grocery outlet on CHURNCREEK RD and follow the road that goes around the building on the LEFT SIDE of grocery outlet. You will see three buildings the third driveway there is a sign for creekside councling turn there to the left. This is the back of the building where the parking is for there office.
