They keep it clean. You must've been in there when it was super busy. It's unfortunate you didn't have a good experience. Nearly everyone that goes loves the place, and goes back many times, which is why it's so busy. The health department checks local restaurants regularly. I'm sure the owner had his certificate displayed.
