You must make a appointment first, you will then be emailed a questionnaire to answer, this questionnaire can also be completed in office before your appointment. Bring to your appointment documentation from your primary doctor or a hospital stating your diagnosis, doctors paperwork can be a few years old however, Your diagnosis must have the word PAIN in it.....or you must have a mental disorder, PTSD is the one they honor..... Just a follow up - any condition that causes chronic pain would make you eligible for medical marijuana. You DO NOT have to have the diagnosis of chronic pain necessarily. Please call us if you have any questions regarding this and we can walk you through the process!
