Casual clothing is okay, but they may have you sit in the bar area, where seating is sort of limited. It's best to have on some decent clothes if you want to go into the actual restaurant, booth area.

Shirt and shoes are certainly expected. Coming in off the bike trail is fine, so fancy dress definitely not required. Enjoy!
I would lean more business casual, but my nursing friend brought clothes to change and they were super helpful with her changing in the restroom.
Casual is fine no tank tops or wife beaters. Just be clean and respectful of others.
