You would have to go in person to the well center it is located at 1401 East Jefferson you would have to go there in person and is open from 8 a.m. to 5 p.m. it is located right behind the church.

Where do I get a application to fill out for rental assistance
You apply online thru this churches forum using their application. It's very difficult to be approved or get a response from them. I've tried 3 times in a month with no response no help. They maybe out if funds.

I'm not aware of this. Haven't been to this church but once since my mother's funeral there in 2014.
