Usually it is all out of pocket. Most dealership charge 89.95 or so to diagnose a vehicle which goes towards the repairs if approved.
At 98,000 miles I don't think Toyota offers any service specials. For the first 2 years or 25,000 miles you would be covered under Toyota Care. Outside of that I think it is all out of pocket.
