Elaine Manley-Chance There isn't any form to transfer a title. Just have her sign the title and send it back. You will register the car in the same way as if you purchased it from a private party. You should bring her plates in to surrender them when you register it.
My mother-in-law now lives in Utah, she is bed ridden. She wants to transfer the title of her car to myself and my son. We have lived with her and driven her car for 12 years. Can you send me the paperwork so I can send it to her in Utah to sign? On the certificate of title her name is on it and my husbands name , to transfer to him after her death. My husband died in 2020.
