As Jess said I would check the website. They will probably ask for a passport. I would also make an appointment and explain to them that you lost your social security card. W2 may work but, without your social security card passport may be asked for. If you know your ssn you they can look it up. As your ssn is your id number no matter what people say.
I don't believe so but you can check the DMV website and it will list the eligible documents for verification and don't forget that you have to schedule an appointment! You cannot just show up.
