I'm going to take a wild guess and assume that they're usually hectic in the back because when things in the back are hectic things get hard to manage. It's a lot to manage when you're always facing a time you're expected to reach then an order for 16 burgers comes in (That example isn't far from truth). There is also a good possibility they're training a new employee every so often, and nobody get's it right the first time. It might seem easy, but fast food is definitely something people need to take time to learn to do it correctly. I'm sure they're trying to fix this problem, but in the meantime remember that you can take your order back, and they seem pretty good about that.
