As above, if you show up during the dinner rush you may have to wait. With a reservation we have always been seated immediately.

Usually if you go around 4 they will take you right in. Can be tough sometimes if its later. We eat there a lot. I recommend calling even if its 1/2 hour before
Not required, but a reservation doesn't hurt, especially if it's prime time on a Friday or Saturday night.
You dont have to. Probably depends on day and time you go. We usually eat dinner earlier than most.
Not sure. We didn't have a reservation when we went, but we got there about 5:30. There were plenty of open tables then but it filled up by the time were were done.
