Yes, they'll have to fill out a short form the first time they go and i think you have to have a black card membership but I'm not 100% sure on that.
Yes. Black Card members can bring a guest at no charge. Guests of White Card members will have to pay $5.
It all depends on what kind of plan you have. If you pay $20 a month most probably yes. $10 a month plan, no.
