call EXT 8898 thats how I got mine
Oh Jeez that hard one. I've been going there a year now. I always ask for a summary detail what I was seen for & what was done, after my visit. But if you need more they won't print it all out because it's to much papers to go through. It's best to have them always sent to your primary Doctor, so sign a release form. But what I have done is, if there's a certain date of important to you, asked the people in the front. & have them look it up to see if it there. & ask for that copy. With me my last appointment, they did a emg test for my damage nerve in my arm. They let me know if I want a copy of the results in paper ask for the copy, because he found it.
Call the main number and ask to speak with medical records. You will probably need to sign a release form. It is handled then by an outside agency and can take some time.
