A shipment got sent out with out the FBA on the boxes and have been received. There doesn't seem to be a phone number to the receiving dock or office at the MDW2 building. How would I go about getting these boxes back or have the FBA added to them?

You may have to go right to the Amazon site itself online . Or the MDW2 LOCATION IS RIGHT ON LARAWSY RD JUST PAST THE RACETRACK .GOOD LUCK
