I believe it cost us $15 and you could buy a book for $5. Instead of buying the book we just had my daughter take the online quizzes. If the license is for a teenager then the parent needs drivers license and proof of residency, and the child needs state certified birth certificate and social security card and one form of proof of residency. If license is for adult you need state certified birth certificate, social security card, two proofs of residency. I used car registration and electric bill.
