You can go online at www.socialsecurity.gov and download/print the for to fill out. You can put original documents for proof they are good at returning it. There is an office in Worthington, Ohio. Make sure you get there at 8:00 am it opens at 9am. People start lining up at 8am. There is a drop of box there so you don't have to wait. They will mail you what you need. Hope this helped. 🤗
