Bring your application along with your current driver's license if you have one, an identification such as your birth certificate or your passport. The most recent bill addressed to you at your current address in PA, however, if you do not have any bills, try to bring a shipment label or a letter showing your name as the recipient with your current address in PA. If you do not have any of them, you may bring someone who are 18 years or older and has a valid identification who can sign a statement to verify that you are who you say you are and that you are living at that address.

A valid identification and a notary signature stating you are who you are and I also believe a a couple bills that are in your name stating where you are living as well. Not sure but as I remember this is what I needed to get my license re issued to me after I lost my wallet.
