You need a birth certificate or passport Original social security card or w2 not more than 18 months old a two proofs of Maryland residency If you're married you will also need original marriage certificate signed by the court clerk We do not take hospitals bills as residents proof Utilities Bill's or bank statements are excepted and we do not expect two proofs of residency from the same place

I would suggest that you bring your birth certificate, utility bill and a passport if you have one. They are very particular.
