If you contact the Pharmacy at 602-263-1506 and tell them your situation, they should be able to reissue the prescription without an additional doctors visit. The phone tree options will be '3' and then '1' to reach the main pharmacy.

Call the pharmacy and let them know you can pick it up. Maybe they can refill it since you didn't pick it up.
