Membership is not required. You get slightly better prices if you are a member. I found that for my occassional purchases it wasn't worth the money. If you are buying for a restaurant and buy many times a month in quantity it pays to be a member. Stop in, shop, talk to them.
I'm gonna say $50. But please don't quote me on it. It's been a while since I paid for the membership.
